Director’s Letter

Thank you very much for your interest in the PGY2 ambulatory care pharmacy residency program at St. John Fisher University and Rochester Regional Health!

The role of the ambulatory outpatient pharmacist continues to grow and evolve, requiring the pharmacist to develop a diverse set of skills in disease set management, practice management, and excellence in communication. Our program aims to train a pharmacist to be able to practice at the top of their license in an outpatient clinical setting in either new or existing practice sites.
Furthermore, graduates will have extensive experience in precepting and teaching, affording them the ability to be excellent preceptors of pharmacy learners and strong teachers in the didactic classroom setting.

At both St. John Fisher University and Rochester Regional Health, we strive to push the profession of pharmacy forward in order to better serve our patients. Throughout the longitudinal rotational experiences, residents will be exposed to disease state management with collaborative drug therapy management, as well as in a consultative role.

The program aims to develop a competent ambulatory care pharmacist in diverse clinical settings. Residents will become proficient in the pharmacotherapy management of most chronic disease states, with an emphasis in diabetes, hypertension, dyslipidemia, COPD, and heart failure. The program also aims to create a competent preceptor and classroom teacher of pharmacy students from the Wegmans School of Pharmacy at St. John Fisher University.

Longitudinal Rotation Sites

TWIG & OPD Internal Medicine at Riedman Health Center

The Riedman Health Center is a brand-new facility featuring state-of-the art technology where the resident has an office location immediately above the clinical patient care areas. The resident spends two days per week here in the interdisciplinary clinics co-managing disease states such as diabetes, hypertension, heart failure, and anticoagulation.

Within this facility is a community pharmacy, laboratory, and other outpatient departments that serve the patients of northern Rochester.

The Sands-Constellation Heart Institute

One day per week, the resident is part of an advanced heart failure management team with a board-certified cardiologist who directs the heart failure program at Rochester Regional Health, nurses, advanced practice practitioners, social workers, and other allied health team members. The resident provides advanced pharmacotherapy services to manage patients with advanced heart failure with complex pharmacotherapy regimens.

Greater Rochester Independent Practice Association

The resident spends one and a half days per week working as an ambulatory care clinical pharmacist on an interdisciplinary care management team to care manage a panel of high risk patients assigned to their care. The resident is expected to promote the rational, cost-effective prescribing of drug therapy for contracted membership by working with patients, caregivers, registered nurses, social workers, advanced practice practitioners and physicians (both GRIPA and non-GRIPA providers). The resident will also enhance and support patient education activities pertaining to chronic disease state management. Residents will also assist in precepting of pharmacy students at the site.

St. John Fisher University Wegmans School of Pharmacy

The resident will participate in the delivery of didactic and experiential pharmacy education to doctor of pharmacy students. As an independent institution in the Catholic tradition of American higher education, St. John Fisher University emphasizes the liberal arts, offering programs in traditional academic disciplines as well as more directly career-oriented fields. The Wegmans School of Pharmacy is a student-centered program aimed at creating patient-centered pharmacists in settings across the health care system.

  • Successful completion of a PGY-1 Pharmacy Residency
  • Licensed as a pharmacist in New York State
